
Before Your Appointment
Please bring with you:
Your dentist or physician can also email us your digital radiograph. If additional films are necessary, they can be taken at our facility.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).


Visit our Forms Page to download patient forms, where you can also upload a completed form. Please print and complete OR fill and upload the “Registration and Health Questionnaire” and the “Informed Consent Form” at home before your first appointment. Billing/Insurance
If we participate in your dental plan, your co-payment is due at the time services are rendered. When you schedule your appointment, please provide us with your detailed insurance information.
For patients without insurance, or if we don’t participate in your dental plan, payment is due at the time services are rendered. Even if we do not participate with your insurance plan, we will submit to your insurance for reimbursement to you as a courtesy.
